Accommodation Based Support

The Federation is part of the iDecide Consortium that provides housing related support in Derbyshire for people experiencing mental ill health. This service is funded by Derbyshire County Council.

Referrals:

The Federation service is accessible to anyone living in the Derbyshire Dales with mental ill health. Services in other parts of Derbyshire are available but would be provided by other members of the consortium. Referrals are accepted from professionals of both statutory and voluntary sector organisations.  Carer and self- referrals are also accepted. The referral form is available from the iDecide website http://idecidederbyshire.org/.

The service:

Housing Support is carried out by an Individual Support Worker (ISW).  All support staff are initially trained to meet Care Certificate standards and then Diploma level 3 in Health and Social Care.  Specialist training is also provided such as budget management, mental capacity act, alcohol and substance abuse, suicide awareness, eating disorders and hate crime awareness.

Following a referral, an assessment is carried out by a Support Manager or trained ISW assessor and if the criteria for Housing support is met using the current criteria from DCC, the client is offered a service and a support action plan is put in place.  This type of support is free of charge.

Clients receiving this support will attend a pre-arranged appointment at our Matlock Wellbeing Hub, with an ISW. Also, clients may be offered the opportunity to attend workshops based on housing related topics, e.g. meals on a budget, fire safety.
